Reporting/Claim Forms

Seattle Colleges centralizes its reporting and recordkeeping to ensure concerns brought forward by staff, faculty, and students are addressed in a timely fashion. Reporting forms can be used for issues including discrimination or harassment, care and concern referrals, and student conduct. Seattle Colleges uses an online incident reporting system called Maxient where anyone can report instances of discrimination or sexual harassment.

Please be aware that these forms are not to be used in an emergency. If this is an emergency, please call your campus security or 911. Additionally, the filing of a report does not constitute filing an official police report.

Bias Incident Report Form—Seattle Central College

Note: to be used only for Seattle Central College. This form is to be used to ensure a timely and effective response to bias-based incidents at any facility or venue owned, operated, or leased by Seattle Central College. A bias-based incident involves conduct—including words, slurs, graffiti, or actions (explicit, implied or perceived) that violates Seattle Central College’s policy prohibiting discrimination.

Discrimination/Harassment/Bullying

This form is for discrimination or harassment complaints based on a protected status, including those based on sex and/or gender (Title IX), race, age, religion, and disability. For a complete list, please see  Policy 282: Discrimination, Harassment and Bullying. This form is also for complaints of bullying. See Procedure 282 for the policy definition of bullying.

Student Conduct/Behavioral Intervention—Seattle Central College

Note: to be used only for Seattle Central College. This form is for student conduct issues as outlined in WAC 132-F-121-110 and/or to report student behavioral concerns that may pose a threat of self-harm or a threat to the community (e.g., alcoholism, suicidal thoughts, family/work stressors).
